If you are a student in Grade 9-11 or have studied in SCLSID High school, and need a TRANSCRIPT, please fill out the form below.
This form is not for G12 students. Please go to E-303 in person or send an email to your college counselor if you need a transcript.
If an unofficial transcript is needed, please go to your Powerschool, print it under ’Student Reports‘.
If you only need a report card, no matter which grade you are in, Please download directly from your Powerschool or contact College Counselor directly, there is no need to fill out this form.
NOTE:
1. Send us your request at least 2 WEEKS before the deadline
2. Please send your request one week before holidays or summer/winter break.

如果您需要非官方成绩单，登录您的Powerschool页面，在“Student Reports”下打印即可。
无论您来自哪个年级，如果仅仅需要Report Card，请直接在Powerschool下载或联系升学老师，不需要填写此表格。
注意：
1. 请至少在截止日期的2周前向升学指导团队提交成绩单报告申请；
2. 请在假期前一周发送相关要求。
